PrimaCare Community Family Health Team (PCCFHT) is very pleased to announce their new partnership with Canadian Mental Health Association, Brant-Haldimand-Norfolk (CMHA).

As a result of this partnership, a CMHA Brant mental health case manager will now be housed with PrimaCare Community Family Health Team at the Willett Site, in Paris, on a regular basis. The services resulting from this collaboration will be to support individuals in the community of Paris and it’s outlying areas in the assessment of their needs and the care coordination and case management of their mental illness. Cindy Bratt, Executive Director for PCCFHT states that “the whole Family Health Team is excited for the community members in Paris who struggle with serious mental illness and have found it very difficult or impossible to get into Brantford to access the services offered by CMHA.”

CMHA is a charitable, non-profit agency, contributing to the mental well being of the community through education and support services to persons 16 years and older with serious mental illness and their families. Mike Benin, Executive Director of CMHA, Brant-Haldimand-Norfolk explains that “CMHA is a team of dedicated mental health professionals, with appropriate educational qualifications and experience working with the seriously mentally ill.” Katrina Sparks, Case Manager is pleased to state that “referrals have already started to come in for members of the Paris Community.” She also explains that “clients can self refer, or ask their Family Doctor to refer them.” Currently, Katrina is in Paris on Mondays, Tuesdays and Wednesdays. She not only sees clients in her office but also visits clients in their homes or in a neutral space if the client prefers. To self refer, you may call 519-752-2998.
