For most of our programs and services a referral is required by your Family Physician. If you see a program or service you would like to participate in please advise your Family Physician or their RN, NP or Physician’s Assistant and they can make the referral for you. The After Hours Clinic is available to patients of the Family Health Team by same-day appointment booking. Please contact your Family Physicians office first to inquire about an appointment. The clinic starts at 5 pm Monday to Friday and 9 am to 12 pm on Saturdays.
Non-Emergency: In the event of a non-emergency medical concern outside regular office hours or on statutory holidays, please contact Health811 by dialing 811 (TTY: 1-866-797-0007) to speak to a Registered Nurse 24/7. A call to Health811 does not replace 911.
Medical Emergency: Please call 911 immediately or go to the nearest emergency department if you or someone else is experiencing a medical emergency.
If you are unable to attend your scheduled appointment or need to reschedule appointments please let us know at least 24 hours in advance. Appointments are in high demand; your prompt cancellation will give another patient the opportunity to see the medical professional.

Missed appointments may prevent other patients from accessing much-needed care. Not adhering to the advance notice policy could result in less sessions with the medical professional.
