Programs

At the PrimaCare Community Family Health Team (PCCFHT), we offer a variety of services and programs to meet the needs of our patients. In addition to the care that you receive from your family doctor, the PCCFHT also has other health care providers available to support your needs. These programs aim to increase the quality of your health, by either helping to manage current conditions or assisting in preventing future health problems.

After Hours Clinic

The After Hours Clinic is for patients of the Family Health Team who need to be seen the same day and cannot be booked in regular office hours. You need to call your Family Physician’s office first and your Family Physician will then refer to the After Hours Clinic if appropriate.

Note: This service is not for routine office appointments or emergencies.

Monday to Friday: 5:00PM – 8:00PM

Saturday: 9AM-12PM

RN After Hour Clinic

At this evening clinic the nurse can see patients for ear syringing, B12 injections, suture and staple removal, well baby checks, well woman exams, sti visits, birth control counseling and memory testing! Call PrimaCare or your family doctor today to book.

This clinic runs between 5-8pm.

Blood Pressure Clinic

At PrimaCare we offer free blood pressure measurement and monitoring sessions. The Blood Pressure clinic is used to promote cardiovascular health in our community and to raise awareness about the importance of blood pressure monitoring. A copy of your results will be given to you and no referral is needed.

Our Blood Pressure Clinic is currently running at 25 Curtis Avenue North in the Community Room on the first floor. It is open to PrimaCare patients and the Community as a drop-in clinic.

Wednesday: 1:30-4pm & Friday 9-11:30am

Chiropody

A Chiropodist assesses and treats foot conditions. Foot problems are very common in people with diabetes and can lead to serious complications. Referral needed from your Family Physician’s office. Our clinic runs on same day bookings Wednesday and Thursdays. Phone lines open at 8:30am.

Some of the conditions a patient may be referred for include:
Subcutaneous lesions
Corns, callouses, warts
Nail conditions

Dietitians

The Dietitian clinic runs on Tuesday and Wednesdays. Registered Dietitians can assist with a broad range of conditions across the lifespan, including but not limited to: type 1 and type 2 diabetes, dyslipidemia, hypertension, irritable bowel syndrome, crohn’s disease, colitis, osteoporosis, infant/toddler/preschooler/school-aged child nutrition, pregnancy, weight loss, weight gain, chronic kidney disease stages 1-3, food insecurity and general healthy eating. Contact your family physician for a referral.

International Normalized Ratio (INR)

For patients who are on Coumadin (also known as Warfarin). These patients are at an increased risk of forming a blood clot or for patients who have experienced a blood clot recently. While these patients are on Coumadin they will be required to have their blood checked on a frequent basis. Our clinic is run by our Pharmacists Natalie and Lindsay. Referral needed from your Family Physician’s office.

Pharmacist

Our Pharmacists work with our patients and provide health-related consultations and specialty health care services and advice. The goal is to help improve the quality of life for our patients. At PrimaCare we believe that these services play a major role in increasing access to primary care and reducing costs to the health care budget. Referral needed from your Family Physician’s office.

Social Work

Assist clients in dealing with mental health challenges by delivering counseling, community services and social support programs. Referral needed from your Family Physician’s office.

This is achieved through: Counseling and psychotherapy

Educating others on the impact of social and environmental factors

Developing, promoting, and delivering programs in collaboration with other professions

PCCFHT Counselling Services provide professional counselling to individuals, couples, families and groups in a safe, confidential and supportive environment.   Therapeutic counselling is a specialized short term service that addresses the psychosocial, emotional, behavioural, relational and mental health needs of individuals, couples and families and is open for PCCFHT patients. Psychoeducational groups are open for both PCCFHT patients and the community.

One day a week in Ayr. 

Psychiatrist

PrimaCare offers Psychiatry services for patients of PCCFHT doctors. Patients 16 and older can be referred by their family doctor for diagnosing and monitoring of various mental health conditions.

RN Foot Clinic

Our RN Foot Clinic runs every Wednesday. At this appointment you will meet with our RN and she will take care of your toenail needs. A referral is needed from your family physician.
